Riverbend officially opened its Fayette Street residence, which provides transitional housing and support services for people with mental health issues, with a ribbon cutting ceremony on July 21, 2009.  Pictured above (from left to right) are:  Louis Josephson, PhD, President & CEO; Commissioner Nicholas Toumpas, Department of Health and Human Services; the Hon. Services Offered:

Fayette is caffeine free and non-smoking, but does have a designated outdoor smoking area.

Staff are present in the residence twenty-four hours each day and are present to work with residents on managing symptoms of their mental illness and to develop social, cooking and housekeeping skills. Three meals and a snack are provided daily for residents. Assistance is given through out the day through supervision and verbal cues to aid in completion of ADL’s and to work together on each individual’s treatment plan and goals.

Admission guidelines/eligibility:
  • You must be at least 18 years old and meet state eligibility.
  • Priority is given to individuals currently residing within Merrimack County or close proximity.
  • You are willing to participate actively in your treatment plan in order to manage symptoms of your mental illness.
  • You will agree to participate in the “Step Up” program (Restorative Partial Hospitalization).
  • You must be able to ambulate independently or with a walker and must be able to manage ADL’s (Actvities of Daily Living skills) with verbal prompts.
  • You must be able to ambulate independently with the use of a walker or wheelchair and must be able to recognize and respond to emergencies such as the fire/smoke alarm with an independent evacuation within 3 minutes.
  • You can not have a diagnosis of dementia or significant memory impairment.
