Project for Assistance in the Transition from Homelessness (PATH) Print E-mail

Riverbend is applying for funding under the PATH program to continue and expand outreach to persons in our service area experiencing homelessness and help them access needed mental health services and co-occurring substance use treatment. We also work to prevent those at imminent risk of homelessness, by helping them connect with mental health services that will help them address issues which may put them in danger of losing their housing. The PATH program and Riverbend work collaboratively with many of the community agencies/organizations to best meet the needs of homeless individuals.

We work with individuals who may have mental illness and/or substance abuse issues. Support is offered to those who are homeless or are in jeopardy of becoming homeless.

We assist in acquiring housing, obtaining access to mental health and other needed services, and help with prevention of eviction.

Services are utilized by Landlords, Property Managers, City and Town welfare personnel, and other mental health agencies and service providers.

The PATH Outreach workers reach out to homeless individuals in their own environments.
