The purpose of the Fayette Street Housing Program is to provide safe, affordable and supportive housing to nine Riverbend consumers who are in need of these services. Residential staff provide functional support services to the resident consumers at this residence for sixteen hours a day.  There is no overnight supervision. The program is intended to assist consumers who, in the absence of functional support services, might otherwise be at increased risk of homelessness and institutionalization. The first floor is handicap accessible. The building is located in the community of downtown Concord, promoting normalization for consumers living at this residence.

Fayette is caffeine free and non-smoking, but does have a designated outdoor smoking area.
Staff are present in the residence sixteen hours each day and are present to work with residents on managing symptoms of their mental illness and to develop social, cooking and housekeeping skills. Assistance is given through out the day through supervision and verbal cues to aid in completion of ADL's and to work together on each individual's treatment plan and goals.

  • You must be at least 18 years old and meet state eligibility.
  • Priority is given to individuals currently residing within Merrimack County or close proximity.
  • You are willing to participate actively in your treatment plan in order to manage symptoms of your mental illness.
  • You will agree to participate in the "Step Up" program (Restorative Partial Hospitalization).
  • You must be able to ambulate independently or with a walker and must be able to manage ADL's (Actvities of Daily Living skills) with verbal prompts.
  • You must be able to ambulate independently with the use of a walker or wheelchair and must be able to recognize and respond to emergencies such as the fire/smoke alarm with an independent evacuation within 3 minutes.
  • You cannot have a diagnosis of dementia or significant memory impairment.
